Year 3 Application

To be considered for payment, participating organizations are required to submit a Year 3 application for the TI 2.0 Program through the AHCCCS Online TI 2.0 Application Portal. Applications must be completed within 30 calendar days of the Portal opening and include identification of Year 3 participating clinics, Year 3 Milestone documentation, and submission of Year 3 attestations, along with any applicable clinic updates.

Organizations need to prepare for the TI 2.0 Year 3 (October 1, 2024 – September 30, 2025) application by completing the following tasks (if applicable):

  1. Organizations must ensure finalized milestone documentation meets the following standards:
    • Documentation must be final policies, protocols, reports, etc. as required in the milestone and document validation.
    • Documents must be named to align with Document Validation naming conventions guidance. Visit the TI Milestone webpage to review Document Validation.
    • Each uploaded document must include a label as appropriate (e.g., Policy; Procedure; Protocol, Report).
    • Documentation must include the provider Organization’s name and/or logo in the header.
    • Documentation must address each milestone element and indicate (e.g., circle or highlight) the areas that address each Milestone element prior to uploading the document. Make sure to include elements from both the Year 3 Milestones and Year 3 Document Validation resources located on theTI Milestone webpage.
  2. Confirm that Organization staff responsible for the Year 3 TI 2.0 Program application can access the AHCCCS Online TI 2.0 Application Portal.
  3. Verify the accuracy and consistency of each participating facility’s information in the AHCCCS Provider Enrollment Portal, ADHS Licensure, and the NPI registry. Specifically:
    • Review NPI guidelines to create a new facility NPI(s) (if needed). If a facility NPI is not needed, add all clinic locations to the “secondary location” of the group/organization NPI in the NPPES NPI Registry. Email the Enumerator for any related questions: customerservice@npienumerator.com. For Year 3, if an organization is adding a new clinic, the organization must review the provider type and ensure it aligns with an area of concentration the organization is already participating in. Organizations cannot add new areas of concentration.
    • Review AHCCCS enrollment via AHCCCS Provider Enrollment Portal (APEP) and ensure effective dates and service addresses are accurate for each facility and individual provider/clinician. Submit an enrollment application for any licensed clinics not already enrolled with AHCCCS
    • Unlicensed PCP facilities only: Review ADHS Health Care Institution Exemption Attestation Form and apply for a facility license(s) as needed.
  4. Confirm that all participating clinics meet the TI 2.0 program eligibility Electronic Health Record (EHR) requirement on the Eligibility & Payment webpage.
    • Electronic Health Record (EHR) System Requirement: No later than 9/30/2025, all participating clinics under the participating Tax ID will implement an EHR system capable of sending and receiving data from Contexture.
    • Health Information Exchange (HIE) Requirement: By 9/30/2025, or within one year of Contexture’s new platform availability (whichever is later), all participating clinics under the participating Tax ID will achieve bi-directional data sharing with the new HIE platform.

Update TI Contacts

Authorized Users and Delegates in Year 3: Only one Authorized Signatory and four Delegates will serve as primary TI 2.0 Program contacts. Delegates are not required. Delegates and Authorized Signatories will be responsible for receiving Payment Information on behalf of the Organization (TIN), in addition to responding to AHCCCS inquiries or audits if applicable. Organizations are able to change the Authorized Signatory and/or Delegates by logging into their AHCCCS Online TI 2.0 Portal at any time.

Year 3 Application Support

Two AHCCCS‑led office hour sessions will be hosted in May and June to support the Year 3 application process. Participants are strongly encouraged to attend to learn more about application requirements and how to navigate the AHCCCS Online TI 2.0 Application Portal. Each session will include a Q&A period, during which Year 3 application questions, milestones, and submission instructions will be addressed.
