How to Enroll with a Health Plan

When you fill out the application, you can pick a health plan. If you do not pick one, we will choose a plan for you. This is referred to as auto-enrollment. If you were auto-enrolled, you will be able to choose a different health plan (if available) within the first 90 days of your enrollment.

When selecting a health plan keep in mind:

  • AHCCCS members may enroll in any health plan that serves their county.
  • All AHCCCS health plans provide the same covered medical services.
  • Before choosing a health plan, check with your doctor, pharmacy, or hospital to see if they are contracted with the health plan of your choice. You can also check by calling the health plan’s Member Services number, or by visiting the health plan’s website.
  • American Indian members may choose to use the American Indian Health Program or may enroll in an AHCCCS Complete Care health plan.
  • If you were enrolled in an AHCCCS Health Plan within the last 90 days but lost your AHCCCS coverage, you will be re-enrolled in your previous health plan.

If you are waiting for your application to be processed, you may still be able to get drug, alcohol, or mental health treatment through other programs run by the Regional Behavioral Health Authority (RBHA). For more information, you can call the RBHA in your area: Central Arizona at 1-800-564-5465, Northern Arizona at 888-788-4408, or Southern Arizona at 866-495-6738. Customers diagnosed with a Serious Mental Illness (SMI);

  • Children in State of Arizona foster care; and
  • Customers who were enrolled with a health plan within 90 days before the new approval, AND that health plan remains available in the customer’s county.

What happens if I don’t pick a plan?

  • If you do not choose a plan, AHCCCS will choose one for you. You will get a Freedom of Choice letter that allows you to change your plan for up to 90 days from the date on the letter.

Any of the following people may choose the health plan:

  • The customer
  • The customer’s legal or authorized representative
  • A responsible adult when the customer is a minor or cannot make decisions

A customer would get the same health plan when:

  • They were enrolled in a health plan within the last 90 days
  • The same plan is still available in the customer’s county
  • Customers diagnosed with a Serious Mental Illness (SMI), and Children in State of Arizona foster care do not get a health plan choice.

How do I choose a health plan?

  • The county in which you live determines what AHCCCS health plan choices are available to you.
  • You can look at Available Health Plans on the AHCCCS website.
  • Each plan has its own doctors, hospitals, and clinics.
  • Choose a plan that includes the doctors or clinics you want.

If you have been enrolled in an AHCCCS health plan within the past 90 days, you will be enrolled with your previous health plan.
