AHCCCS Provider Survey Open Through May 17, 2021

AHCCCS is conducting a survey to determine the performance and responsiveness of its contracted managed care organizations (MCO) to providers. We are asking providers who are contracted with the health plans listed below to complete this survey by May 17.

Responses will help AHCCCS determine levels of satisfaction with MCO Claims, Customer Service, Credentialing and Prior Authorization processes.

Providers contracted with the following MCO’s are asked to participate:

• Arizona Complete Health-Complete Care Plan
• Banner University Family Care
• Care 1st Health Plan Arizona
• Health Choice Arizona
• Magellan Complete Care
• Mercy Care
• UnitedHealthcare Community Plan

Limited provider information will be collected as part of this survey. AHCCCS does not intend to release details identifying individual provider submissions.

This feedback is very important for ongoing assessment and improvement.
