The AHCCCS Solutions Center (ASC) will be temporarily unavailable on Sunday, November 8, from 3:00 AM to 9:00 AM Arizona Time due to a planned system upgrade. ALTCS Member Survey Open Through February 4

The Sonoran Center for Excellence in Disabilities, in collaboration with AHCCCS, is surveying ALTCS members in long term care settings and their families. The survey asks questions about whether members feel safe and protected. Individual answers will not be shared.

Please share this survey link with stakeholders, advocates, and ALTCS members.

The survey is available in English and Spanish through February 4, 2022.
