Promising Practices is designed to encourage tribal communities, Indian Health Services, tribally operated 638 facilities and others to share programs that have a positive impact on tribal communities in one of the following four areas highlighted below.
Areas of Promising Practices
Prevention & Chronic Disease-Management - The goal of Prevention is to prevent chronic disease in the first place. Once someone has been diagnosed, Chronic Disease Management aims to lessen the severity and improve the health-related quality and duration of the individual's life. Heart disease, stroke, cancer, diabetes, and asthma are a few examples of some of the most common chronic diseases.
Behavioral Health - Behavioral health services include evaluation and treatment and support services for individuals with mental illness and/or substance abuse.
Care Coordination - One of the greatest challenges with health care today is that providers do not talk to one another. A primary care provider, for instance, may have no idea that their patient has gone to the Emergency Department 5 times in the last few months. There is health care but not a system of care. This impacts the quality of care received. Care coordination is about building a team approach to health care by connecting providers to each other, their patients and the community. By providing health care in a coordinated fashion, we can improve outcomes and reduce health related disparities.
Elder Services & Long Term Care - Arizona has been a leader in providing home and community based services to elders and individuals with physical or cognitive disabilities. This means we are able to keep people out of institutions, which provides for better quality of life for our members.
