AHCCCS Informs Members of Data Breach, Implements Immediate Corrective Measures

The Arizona Health Care Cost Containment System (AHCCCS) is notifying 3,177 members of a data breach that occurred on August 29, 2025, involving misaddressed member communications.
The incident occurred during a routine mailing to members regarding their health plan enrollment. Later that day, AHCCCS was alerted by a member who received a letter addressed to a different individual. AHCCCS immediately halted its mailing process and launched an internal investigation.
The breach resulted in the disclosure of limited protected health information (PHI) including member names, AHCCCS identification numbers (A#), and health plan names. No Social Security numbers, financial data, or clinical information were involved.
AHCCCS has taken the following steps to mitigate the issue, including:
- Halting the mailing process immediately upon discovery.
- Conducting a thorough review of the mailing system and procedures.
- Notifying affected members and offering guidance on how to protect their information.
AHCCCS deeply regrets this error and remains committed to safeguarding member data. In accordance with federal and state privacy laws, AHCCCS has taken all necessary steps to assess the scope of the breach, notify affected individuals, and prevent future incidents. Affected members who are concerned about potential misuse of their personal information may consider the following steps:
- Place a fraud alert with one of the three major credit reporting agencies to flag potential identity theft.
