Long Term Care provided for American Indians living on a reservation is known as the Arizona Long Term Care System (ALTCS) Tribal Case Management Program. American Indians living off-reservation may choose a Long Term Care health plan that serves their county of residence.
The Arizona Long Term Care System (ALTCS) Tribal Case Management Program provides acute care (general medical care services), behavioral health, long term care, and case management services. These services are offered to American Indian ALTCS members at risk of institutionalization (i.e. those who are elderly and/or have physical disabilities) who reside on a tribal reservation.
AHCCCS contracts with seven tribes to provide case management for ALTCS members. The seven tribes and urban Indian health program, commonly referred to as "tribal contractors", provide case management services for their enrolled ALTCS members. These tribal contractors are listed in the following table.
American Indian ALTCS members residing on reservations other than the seven tribes receive case management services through the Native American Community Health Center (phone: 602-297-5262).
