HB 2800 Provider Attestation

In accordance with HB 2800, AHCCCS is prohibited from contracting with providers who perform certain types of abortions. Only providers or facilities qualified to perform abortions within the scope of their license will be required to complete and return the attestion. AHCCCS will send the attestation form to selected providers by mail. Once a provider has received the form, then he or she is required to complete it and return it to AHCCCS provider registration, as the letter will direct. For facilities that receive the attestation, the attestation should be completed by someone with the authority to bind the provider (e.g., the principle or an officer of the corporation).
