Physician Fee Schedules

Physician fees schedules are updated annually, on or near October 1st of each year. Quarterly adjustments January 1, April 1, and July 1 may be made to accommodate new codes or rate adjustments.

The appearance on this website of a code and rate is not an indication of coverage, nor a guarantee of payment. AHCCCS covered procedures can be viewed in the AHCCCS Medical Policy Manual (AMPM). AHCCCS covered services can differ based upon enrollment.

  • FFS/Acute MCO/ALTCS MCO Fee Schedules Effective October 1, 2009
  • Acute MCO Program Capped Fee Schedule Effective May 1, 2009
    This fee schedule contains the reimbursement rates that contracted Acute Health Plans should index for reimbursement rates that are based on AHCCCS Physician Fee Schedule rates. For the rates that were in effect prior to May 1, 2009, reference Acute MCO Program Capped Fee Schedule Effective January 1, 2009.
  • Acute MCO Program Capped Fee Schedule Effective January 1, 2009
    This fee schedule contains the reimbursement rates that contracted Acute Health Plans should index for reimbursement rates that are based on AHCCCS Physician Fee Schedule rates. For the rates that were in effect prior to January 1, 2009, reference Physician Fee Schedule Effective October 1, 2008.
  • ALTCS MCO Program Capped Fee Schedule Effective January 1, 2009
    This fee schedule contains the reimbursement rates that contracted ALTCS Health Plans should index for reimbursement rates that are based on AHCCCS Physician Fee Schedule rates. For the rates that were in effect prior to January 1, 2009, reference Physician Fee Schedule Effective October 1, 2008.
  • FFS Program Capped Fee Schedule Effective February 1, 2009
    This fee schedule contains the reimbursement rates that AHCCCS pays when AHCCCS is the payer. For the rates that were in effect from January 1, 2009 thru January 31, 2009, reference Acute MCO Program Capped Fee Schedule Effective January 1, 2009. For the rates that were in effect prior to January 1, 2009, reference Physician Fee Schedule Effective October 1, 2008.
