  For the period 10/01/2012 - 09/30/2013, the ASC Cost-to-Charge Ratio is: 0.1834.
• Rates Effective February 1, 2012 [Excel, 716KB]
  

  Effective February 1, 2012 the AHCCCS ASC Fee Schedule amounts are calculated using 80% of the CY 2012 CMS proposed ASC Fee Schedule issued under CMS 1525-P which was the most current version available as of September 2011. Procedure codes that include a device or implant cost are adjusted to include the full device or implant cost using the CMS OPPS Device Offset amounts.
