Overview of the ALTCS Tribal Case Management Program

The Arizona Long Term Care System (ALTCS) Tribal Case Management Program provides acute care (general medical care services), behavioral health, long term care, and case management services. These services are offered to American Indian ALTCS members at risk of institutionalization (i.e. those who are elderly and/or have physical disabilities) who reside on a tribal reservation.

ALTCS Tribal Contractors

Seven tribes have an Intergovernmental Agreement (IGA) with AHCCCS to provide case management services to their respective tribal members enrolled in ALTCS. The seven tribes and urban Indian health program commonly referred to as "tribal contractors" provide case management services for their enrolled ALTCS members. These tribal contractors are listed in the following table.

American Indian ALTCS members residing on reservations other than the seven tribes receive case management services through the Native American Community Health Center (phone: 602-297-5262).
