Tips for Searching:

This form will return a list of matching records for the criteria you enter. You may search for a specific code, or description. You may also search with a partial code or description, or both a partial code and partial description. Any fields left blank will be ignored. 

If you're not sure of the exact code or description, just type in the first letter (or number) of the code, or a word in the description, and records will be returned which begin with the letter or letters which you entered or contain the word you entered in the description. 

Examples:

IF you enter... THEN the search will return...
a Code of "080" and a blank Description field (without the quotes) all records that contain "080" somewhere in the code.
a Description of "Mental Health" and a blank Code field (without the quotes) all records that contain "Mental Health" somewhere in the description.
a Code of "080" and a Description of "Mental Health" (without the quotes) all  records that contain *both* "080" somewhere in the Code field AND "Mental Health" somewhere in the Description.
