When did the additional requirements start?
July 1, 2006.

Who does this new law affect?
All AHCCCS and KidsCare members and applicants declaring U.S. citizenship.

How does the new law affect AHCCCS applicants?

  • U.S. citizens applying for AHCCCS must document citizenship and identity during the application process. Applicants will not be approved until all documentation is provided to AHCCCS or DES.
  • AHCCCS/DES can help verify citizenship for applicants born in Arizona.

How does the new law affect AHCCCS members?

  • U.S. citizens with AHCCCS health insurance must document citizenship and identity during their next review.
  • AHCCCS or DES will send a letter to members before their next review with details about the additional documentation requirements.
  • AHCCCS/DES can help verify citizenship for members born in Arizona.

Who is not affected by the new law?

  • Members receiving SSI Cash or Medicare do not have to provide additional documentation to AHCCCS or DES, as U.S. Citizenship is already verified through the Social Security Administration.
  • Non-citizens are not affected, as they already provide documentation of immigration or legal status.
  • Federal Emergency Services (FES) members are not affected, as federal law does not require citizenship for these services.

When should members submit documents to AHCCCS and/or DES?

  • Members should submit original documents or certified copies at the time of review.
  • Original documents SHOULD NOT be mailed to AHCCCS or DES!

What documents are required?
All AHCCCS members and applicants declaring U.S. citizenship must provide the most reliable proof of citizenship and identity available. Lists 1 through 4 below show the levels of acceptable documentation.

Document your U.S. citizenship & identity with:

Most Reliable
  • United States (U.S.) Passport
  • U.S. Naturalization Certificate issued by U.S. Citizenship & Immigration Services (USCIS)
  • Certificate of Citizenship issued by USCIS
Document your U.S. citizenship with:
List #2
  • U.S. Birth Certificate
  • Certification or Report of Birth Abroad issued by USCIS or the State Department
  • U.S. Citizen ID card issued by USCIS
  • American Indian card issued by USCIS for the Kickapoo tribe
  • Final Adoption Decree
  • Evidence of Civil Service employment by U.S. Government before 6/1/1976
  • Official Military Record of Service showing U.S. place of birth (i.e. a DD-214)
  • Northern Mariana ID card issued by USCIS to a naturalized citizen born before 11/4/1986

  • Extract of U.S. hospital birth record established at the time of birth
  • Life, health, or other insurance record showing U.S. place of birth
(Either of these must have been created at least 5 years before the initial AHCCCS application date.)

Least Reliable
  • Census records showing U.S. place of birth
  • Nursing home records, medical records, or other documents showing U.S. place of birth created at least 5 years before initial AHCCCS application date
  • Bureau of Indian Affairs tribal census records (Navajo & Seneca tribes only)
  • Written affidavit (see eligibility specialist for details)
Document your identity with:
  • Driver's License
  • Certain government issued ID cards with photo (if no photo, must include identifying information)
  • Tribal government issued ID and documents, including Certificate of Indian Blood
  • Day care or nursery record (minors only)
  • School record or report card (under 16 only)
  • School ID with picture
  • U.S. Military ID, U.S. Military Dependent ID or U.S. Military Draft Record (over 16 years only)
  • U.S. Coast Guard Merchant Mariner Card
  • Affidavit (under 16 only)

What if a member or applicant needs help obtaining documentation?

  • For Birth Certificates (born in Arizona): AHCCCS and DES can help verify Arizona births with specific information. Contact the local office for further information.
  • For Birth Certificates (born outside Arizona): Contact Vital Records in the state of birth. For state contact information go to http://www.azahcccs.gov
  • For Naturalization Documents: Contact the USCIS on the web at http://www.uscis.gov, or by phone at 1-800-375-5283
  • For other information and resources online go to http://www.az211.gov.
