Eligible Professionals (EP) Logo

AHCCCS has been granted the authority to extend the Program Year 2014 attestation tail period through June 30, 2015 for Eligible Professionals. This is largely in order to accommodate the state’s implementation plan for the Flexibility Rule, which is scheduled to be available for provider attestations in early May 2015.

Medicaid EPs include:

  • Physicians
  • Nurse Practitioners
  • Certified Nurse - Midwife

To assist you with registering, AHCCCS has prepared a Toolkit for Eligible Professionals. The toolkit contains:

    Establish Practice Request Form
[Excel] is used when Eligible Professionals (EPs) in a Group Practice or Clinic would like to attest using the Practice’s data to qualify for the EHR Incentive Program’s patient volume criteria. The Aggregate Patient Volume Methodology uses the Practice’s patient encounters for the entire Practice (multiple providers) but can only be used as a proxy for all EPs in the Practice if all the Mandatory Federal and State Specific Rules are met.
