AHCCCS provides a checklist that must be completed and submitted by a medical facility/entity or professional (e.g. hospitals, medical facilitates, and health care professionals) that makes or receives Medicaid payments of at least $5 million dollars annually.
The checklist is used to certify compliance with Section 6032 of the Deficit Reduction Act.
