Capitation Rate Submission Program Instructions/Database

Note: It is important to perform the following steps to use the Capitation Rate Submission Program:

1. Click on the Instructions... link below and read through the instructions.
2. Then, click on the ALTCS RFP Database link.
3. Next, click Save in the dialogue box that is displayed to save the database to your own PC.
4. Open the saved database and follow the prompts.

Note: On the General Info template, the field labeled "Contract Information" should be labeled "Contact Information" Please complete this field with the name of the person to contact with any questions regarding the submission. (Posted: March 28, 2006)
